What makes live chat HIPAA compliant?
Live chat is usable for protected health information when the vendor signs a Business Associate Agreement (BAA) with you and you configure and use the tool within that agreement. No chat tool is "HIPAA compliant" on its own: compliance depends on the vendor's safeguards, the plan and features covered by the BAA, and how your team uses it.
Under HIPAA, a vendor that creates, receives, maintains or transmits PHI for a covered entity is a business associate, and HHS publishes sample BAA provisions. A patient typing a date of birth and symptoms into a website chat is enough to bring the chat vendor into scope.
This list covers live agent chat and secure messaging for clinics and health plans. If you mainly want an AI chatbot, see our HIPAA-compliant chatbots list.

How did we compare these tools?
We only included tools whose own site, pricing page or help center says they sign a BAA or list HIPAA support. We then compared them on what a clinic or health plan actually buys.
- BAA scope: which plan or add-on you need before PHI can enter the tool, and whether the BAA is signed online or negotiated.
- Live agent workflow: chat routing, shared inbox, transfers and whether chat sits next to phone, SMS and email.
- AI with guardrails: whether AI can take routine admin questions and hand clinical topics to people.
- Real cost: the price of the HIPAA-eligible tier, not the cheapest plan on the page.
- Fit: small practice, multi-site clinic group, telehealth, or health plan member services.
We left out tools that say they do not sign BAAs. Aftersales is our product and we list it first; each entry, including ours, says where it falls short.
The 9 best HIPAA compliant live chat tools
1. Aftersales: best for health plans and clinics that want live chat, a shared inbox and AI in one place
Aftersales puts live chat, email, SMS, WhatsApp and phone into one Inbox that human agents and AI share. For HIPAA work the important detail is the plan: HIPAA support with a signed BAA is on the Enterprise plan (custom pricing, contact sales). Free, Starter, Growth and Scale do not include a BAA, so do not put protected health information on them.
Human agents get Copilot, which drafts replies that cite their sources, summarizes long threads and translates in 31 languages. That helps member services teams answer the same benefits or billing question consistently. Agent can take the routine administrative share, such as how to update an address or where to find a form, and hands off to a person with the conversation and what it already tried attached. Agent should not give medical advice; set escalation rules so clinical questions, distress and complaints go straight to staff.
Where a specialist is better: Aftersales has no native connection to EHR or practice management systems. Those connect through the Aftersales API or webhooks, so confirm specific connectors on a call. A small practice whose main need is patient texting and phones tied to the schedule will usually be better served by Spruce, Weave or ModMed Patient Engagement below.
- Best for
- Health plans, telehealth and multi-site clinics running a member or patient services team across chat, email, SMS and phone
- Pricing
- Free, Starter $24, Growth $69 and Scale $109 per seat/month billed annually (20% more monthly). HIPAA support with a signed BAA is on the Enterprise plan only (custom pricing). Agent $0.99 per resolution, counted once per conversation; conversations it can't help with are free. Copilot add-on $29 per rep/month annual.
- Strengths
- One inbox for chat, email, SMS, WhatsApp, social and phone; Copilot drafts with sources; Agent with escalation rules on sentiment, keywords and customer tier; quality Monitors on every conversation; SSO/SAML/SCIM on Scale; GDPR DPA and US-only hosting
- Watch out for
- BAA requires the Enterprise plan (custom pricing). All data is hosted in the US only. No native EHR or practice management integration (API and webhooks only). Not a patient-texting or scheduling tool for small practices.
2. Comm100: best for large live chat teams that want a HIPAA-focused chat vendor
Comm100 is a long-standing live chat and omnichannel vendor with a dedicated healthcare page. Its pricing page lists HIPAA compliance as a feature of Live Chat Ultra, and its healthcare page tells buyers to review the Business Associate Agreement process with their privacy, security and legal teams. Chat routing, audio-video chat and chat wrap-ups appear from the Plus tier.
- Best for
- Hospital systems, insurers and other large teams that want a mature live chat console with HIPAA scope
- Pricing
- Live Chat Startup $31 and Live Chat Plus $55 per agent/month billed annually ($39 and $69 monthly). Live Chat Ultra, the tier that lists HIPAA compliance, is quote-only.
- Strengths
- Deep live chat feature set (routing, wrap-ups, campaigns, audio-video chat), plus AI Agent, ticketing and knowledge base products; HIPAA, SOC 2 Type II and other compliance listed on the pricing page
- Watch out for
- HIPAA sits on the quote-only Ultra tier, so the published prices are not what a HIPAA buyer pays. Confirm which Comm100 products are covered by the BAA.
3. LiveChat: best for teams that want a familiar chat widget with a BAA at enterprise level
LiveChat (by Text) is one of the best-known website chat tools. Its pricing page lists HIPAA Compliance as an Enterprise plan feature and says HIPAA support requires a signed BAA, available on eligible plans on request.
- Best for
- Organizations that already like LiveChat's agent app and only need chat plus ticketing
- Pricing
- Starter from $19, Team from $49 and Business from $79 per month (per seat, billed annually) on the pricing page. HIPAA compliance is on Enterprise, which is quote-only.
- Strengths
- Polished chat widget and agent app, large integration marketplace, SSO and SLAs on Enterprise
- Watch out for
- The plans with public prices do not include HIPAA. Chat-first, so phone and SMS need other tools.
4. Zendesk: best for health organizations already standardized on Zendesk
Zendesk publishes a Business Associate Agreement that subscribers can sign. On its pricing page, HIPAA enablement is an add-on for Suite Professional and included as "HIPAA enabled" on Suite Enterprise. Zendesk points to its Advanced Compliance documentation for the list of services that can be HIPAA-enabled, so check that messaging and live chat are on it for your setup.
- Best for
- Larger teams that want ticketing, messaging, voice and help center from one vendor and can buy at the upper tiers
- Pricing
- Suite Team $55, Suite Professional $115 per agent/month billed annually; Suite Enterprise is quote-only. HIPAA is an add-on on Professional and included on Enterprise.
- Strengths
- Mature ticketing, routing and reporting; very large app marketplace; department spaces and access logs on upper tiers
- Watch out for
- HIPAA scope depends on plan, add-on and which Zendesk services you use. AI agent pricing is separate; confirm whether AI features are inside your BAA.
5. Intercom: best for digital health and SaaS-style health companies that live in a messenger
Intercom lists HIPAA support as an Expert plan feature on its pricing page, and its help center says it can enter into a BAA with customers on a plan that offers HIPAA support. Intercom's Messenger suits app-based health products where members chat inside the product.
- Best for
- Digital health apps and telehealth startups that want in-app messaging
- Pricing
- Essential $29, Advanced $85, Expert $132 per seat/month on the pricing page. HIPAA support is on Expert. Fin AI agent is $0.99 per outcome.
- Strengths
- Strong in-app Messenger, product tours and help center; Fin AI agent available
- Watch out for
- BAA only on the top plan, so every seat pays the Expert price. Request the BAA before any PHI enters the workspace.
6. Freshchat: best for budget-conscious teams that can follow a strict configuration guide
Freshworks says it may sign a BAA for Freshchat customers who are covered entities or business associates, and publishes a HIPAA configuration guide for standalone Freshchat. The guide sets a mandatory operating environment; for example, it tells customers to turn off visitor push notifications because those go through sub-processors that do not sign a BAA. Freshchat's published Enterprise plan is where features such as audit logs and IP allowlisting sit.
- Best for
- Smaller member or patient services teams that want a lower per-agent price and have IT capacity to configure it
- Pricing
- Free (up to 10 agents), Growth $19, Pro $49, Enterprise $79 per agent/month billed annually. Freddy AI Agent sold in session packs.
- Strengths
- Low entry price, web and mobile chat widget, shared inbox, published HIPAA configuration guide
- Watch out for
- BAA is case by case and the guide covers standalone Freshchat only. Confirm the plan Freshworks requires before you sign.
7. Spruce Health: best for small practices that want secure messaging, phone and fax in one app
Spruce is a communication platform built for healthcare. Its pricing page lists a HIPAA BAA on every plan and says trials include one, accepted electronically when you create an organization. Spruce combines phone, text, secure messaging, video, fax and a shared inbox. Instead of a classic chat widget, practices share a Spruce Link from their website so patients can start a secure conversation.
- Best for
- Independent and small group practices, therapists and direct primary care
- Pricing
- Basic $24 and Communicator $49 per user/month, month-to-month by default. A one-time $19.50 fee applies to enable outbound SMS.
- Strengths
- BAA on every plan and trial; built-in phone, fax, video and secure messaging; low cost per user
- Watch out for
- Secure messaging and video require patients to use the Spruce app. Not designed for large contact centers or AI-resolved chat.
8. Weave: best for dental and specialty offices that want web chat to become a text thread
Weave is a patient communication platform for healthcare offices. Its Text Connect widget lets website visitors start a text conversation with the office rather than a browser chat session, so the conversation continues on the patient's phone. Weave publishes a Business Associate Addendum on its legal pages and pairs messaging with phones, reminders, reviews and payments.
- Best for
- Dental, optometry, veterinary and other front desks where phone and text matter more than browser chat
- Pricing
- Plans start from $199 per month; higher tiers are quote-based.
- Strengths
- Website-to-text widget, phones and texting in one system, published BAA, front-desk focus
- Watch out for
- Not a live chat console for a large team. Pricing is per location bundle, which can be more than a small chat-only need requires.
9. ModMed Patient Engagement (Klara): best for specialty practices that want call-to-text and secure threads
Klara is now sold as ModMed Patient Engagement powered by Klara. It converts calls and transcribed voicemails into encrypted text chats, offers app-free two-way texting, online scheduling and intake, and includes AI summaries of chats. It is aimed at specialty practices such as dermatology, gastroenterology, ophthalmology and orthopedics.
- Best for
- Specialty medical practices that want to move phone volume to secure text
- Pricing
- Not published; request a demo.
- Strengths
- Call-to-text, encrypted patient messaging without an app or password, AI chat summaries, scheduling and intake
- Watch out for
- Built for practices, not health plans or large contact centers. Confirm the BAA and EHR integration scope with ModMed.
Which HIPAA compliant live chat tool is best for your team?
- Health plan or payer member services: Aftersales (Enterprise) for chat, email, SMS and phone in one inbox with Copilot and Agent; Zendesk Enterprise if you are already on Zendesk.
- Telehealth and digital health apps: Aftersales or Intercom Expert. See our telehealth AI customer service page.
- Hospital or insurer contact center focused on web chat: Comm100 Live Chat Ultra or LiveChat Enterprise.
- Small independent practice: Spruce Health. A BAA on every plan and phone, fax and messaging for $24 to $49 per user is hard to beat; Aftersales is the weaker fit here.
- Dental and optometry front desks: Weave, because patients start on the website and continue by text. See also AI customer service for dental practices.
- Specialty medical practices with heavy phone volume: ModMed Patient Engagement (Klara) for call-to-text.
- Tight budget with IT support: Freshchat, if Freshworks confirms the BAA for your plan and you follow its configuration guide.
What should you check before putting PHI in live chat?
- Get the BAA signed before any PHI enters the tool, including during a trial.
- Confirm which products, channels and AI features the BAA covers. Push notifications, third-party apps and integrations are common gaps.
- Turn on SSO, role-based access and audit logs, and limit who can export transcripts.
- Set retention and redaction rules for transcripts and attachments.
- Write escalation rules: clinical questions, emergencies and distress go to people, and AI never gives medical advice.
- Add a short notice in the widget telling patients not to share emergency information and where to call instead.
- Review a sample of conversations every week. In Aftersales, Insights Monitors score every conversation rather than a sample.
