What should customer service software for healthcare do?
Customer service software for healthcare is a helpdesk or contact center that a provider, health plan or health brand can use with patient data. In practice that means four things: the vendor signs a business associate agreement (BAA) covering the channels and AI features you plan to use, the product has the access controls and audit trails your security team needs, it handles the channels patients actually use (phone and text as much as email and chat), and it keeps clinical questions with licensed staff.
Under HIPAA, a vendor that stores or transmits protected health information (PHI) for you is a business associate. HHS explains this in its business associates guidance, and its cloud computing guidance says a cloud service handling ePHI needs a BAA. The HIPAA Security Rule sets the safeguards (access control, audit controls, transmission security), and the minimum necessary standard is why agents should see only the patient data a conversation needs.
This list covers full helpdesks and contact centers for patient services, member services and support teams. If you only need a bot or a chat widget, see our lists of HIPAA-compliant chatbots and HIPAA-compliant live chat. No software makes you compliant by itself. Your configuration, policies and training do that, so confirm your obligations with your compliance team.

How did we compare these tools?
We looked at tools that a healthcare team can run its whole patient or member service operation on, not just a bot. Every claim below comes from the vendor's own pricing page, help center or legal documentation, checked in October 2026. We compared them on:
- BAA terms: which plan or add-on you need, and which channels and AI features sit inside the covered services.
- Channels: phone and SMS carry much of healthcare's volume, so a helpdesk without them needs a second tool.
- Safeguards: role-based access, field-level protection for PHI, audit logs, SSO.
- AI for staff and patients: reply drafts and summaries for agents, and an AI agent for routine administrative questions, with a clean handoff.
- Connecting healthcare systems: whether EHR, practice management and scheduling data reach the agent's screen natively or through an API.
- Pricing: the published price of the plan that includes the BAA, not the entry plan.
Aftersales is our product and it is listed first. Every entry has a "watch out for", including ours, and we say plainly where a specialist fits part of healthcare better.
The 9 best customer service software options for healthcare in 2026
1. Aftersales: best for patient and member service teams that want an AI agent and a helpdesk under one BAA
Aftersales is an AI customer service platform. Inbox gives staff one queue for email, web and in-app chat, WhatsApp, SMS, phone, social DMs and Slack Connect, with views, SLAs and side conversations into Slack, so a billing question can go to finance without leaving the thread. Agent resolves routine conversations end to end, and Copilot drafts replies that cite their sources, summarizes long threads and translates in 31 languages, including right-to-left scripts. HIPAA support with a signed BAA is on the Enterprise plan; Scale adds SSO, SAML and SCIM and a dedicated success manager. See the security page.
It fits the administrative side of healthcare support: billing and payment questions, appointment logistics, portal access, prescription delivery status, and subscription or order questions for telehealth and direct-to-patient brands. You write each procedure in plain language, and escalation rules hand a conversation to a person on sentiment, keywords, customer tier or legal language. Agent should never give medical advice; set it to pass symptoms, medication and diagnosis questions to licensed staff with the conversation and what it already looked up attached. Insights runs quality Monitors on every conversation instead of a sample.
Where a specialist is better: Aftersales has no native EHR or practice management integration. Those systems connect through the Aftersales API or webhooks, so confirm specific connectors on a call. A health system whose main problem is call volume into a central access center will get more from a healthcare contact center such as Talkdesk, and a small practice that mostly needs phones and patient texting will be well served by Spruce.
- Best for
- Telehealth, digital health, multi-site clinics, health plans and health brands running patient or member services across email, chat, SMS, WhatsApp and phone
- Pricing
- Free, Starter $24, Growth $69 and Scale $109 per seat/month billed annually (20% more monthly); Enterprise is custom. The BAA is on Enterprise only. Agent $0.99 per resolution, counted once per conversation; conversations Agent can't help with are free. Copilot add-on $29 per rep/month annual. Free Lite seats (50 on Scale) for clinicians or managers who only need to view, comment and approve.
- Strengths
- AI agent and helpdesk in one product, plain-language procedures with spend limits and escalation rules, Copilot drafts with sources, Monitors on every conversation, US hosting, SSO/SAML/SCIM on Scale. Median 71% of conversations resolved by Agent after 90 days.
- Watch out for
- BAA requires the Enterprise plan. No native EHR, scheduling or practice management connectors (API and webhooks only). Not a clinical triage tool. Using Aftersales does not by itself make you compliant.
2. Zendesk: best for large support teams already standardized on Zendesk
Zendesk lets accounts on Suite Professional, Enterprise or Enterprise Plus buy the Advanced Compliance add-on and sign a BAA. Its documentation lists ticketing, help center, community, chat and messaging, analytics and Talk (voice, excluding text) as HIPAA-enabled, plus add-ons including Copilot, AI Agents - Advanced, Contact Center and Workforce Management. Early access programs, Built by Zendesk marketplace apps and social media integrations are excluded. Its pricing page shows "HIPAA enabled" on Suite Enterprise.
- Best for
- Established support teams with admins who can configure a large helpdesk and want voice, ticketing and help center from one vendor
- Pricing
- Suite Team $55 and Suite Professional $115 per agent/month billed annually; Enterprise via sales. Advanced Compliance add-on price not published. AI agent resolutions beyond the included allowance are $2.00 each pay-as-you-go.
- Strengths
- Mature ticketing, routing and reporting, a published list of HIPAA-enabled services and add-ons, very large app marketplace
- Watch out for
- Costs stack across plan, compliance add-on and AI. SMS is outside the Talk coverage. Required settings after signing include enforced 2FA and authenticated attachment access. Compare with our Zendesk alternative page and Zendesk pricing.
3. Salesforce Health Cloud: best for health systems and payers that want service inside a healthcare CRM
Health Cloud is Salesforce's CRM for providers and payers, built on Service Cloud. Its editions include case management, omni-channel routing, a service console, care team collaboration and referral management. Salesforce lists Health Cloud and Service Cloud among the services covered by its HIPAA Business Associate Addendum. Its BAA restrictions page says Einstein services are not a substitute for professional medical advice and that PHI must not be submitted in Einstein Bot utterance records.
- Best for
- Health systems, payers and life sciences companies that want patient or member service tied to a full CRM and care coordination
- Pricing
- Health Cloud Sales & Service Core $350, Advanced $525, Service Max $700 and Sales & Service Max $725 per user/month, billed annually. Agentforce Contact Center (Digital) is an add-on on the Max editions.
- Strengths
- Healthcare data model, case management and routing, care coordination, broad covered-services list, Agentforce AI with documented restrictions
- Watch out for
- The most expensive option here per user, and usually needs an implementation partner. Read the service-specific BAA restrictions before turning on AI. See Zendesk vs Salesforce Service Cloud.
4. Talkdesk Healthcare Experience Cloud: best for phone-heavy patient access centers
Talkdesk sells a contact center built for healthcare providers and payers. Its AI agents handle appointment scheduling, prescription refills, patient identification and portal access, post-discharge engagement and care navigation, alongside routing (Navigator), agent assist (Copilot) and quality management (QM Assist). Talkdesk says the platform gives agents a unified view of EHR, CRM and other systems and handles PHI in a HIPAA-compliant, encrypted way.
If most of your volume is phone calls into a scheduling or access center, Talkdesk is a better fit than Aftersales. It was built voice first for that problem.
- Best for
- Hospitals, health systems and payers running large voice contact centers with scheduling and refill workflows
- Pricing
- Industry Experience Clouds, which include Healthcare, are listed at $225 per user/month. Digital Essentials $85, Voice Essentials $105 and Elite $165 per user/month. Billing terms through sales.
- Strengths
- Voice-first contact center, healthcare AI agents, workforce and quality management, omnichannel (voice, SMS, email, chat, app)
- Watch out for
- Built for contact center scale, so it is more than a small team needs. Confirm BAA scope and which EHRs connect for your setup.
5. Freshdesk: best for budget-conscious teams that will follow a strict configuration guide
Freshworks says its BAA scope covers Freshdesk, Freshchat, Freshcaller and Freshdesk Omnichannel, and that it does not recommend processing ePHI in its other products. Its HIPAA guidance sets a secured operating environment: careful call recording settings, role-based access, SSL and hardened end points. The pricing page does not say which plan the BAA requires.
- Best for
- Smaller patient or member service teams that want ticketing, chat and phone from one vendor at a lower per-agent price
- Pricing
- Growth $19, Pro $55 and Enterprise $89 per agent/month billed annually. Freddy AI Agent includes 500 sessions, then $49 per 100 sessions. Freddy AI Copilot $29 per agent/month on Pro and Enterprise.
- Strengths
- Low entry price, ticketing plus chat and phone in the BAA scope, published configuration guidance
- Watch out for
- Freshworks' HIPAA page does not say whether Freddy AI is covered, so get that in writing. Confirm the plan required for a BAA. See our Freshdesk alternative page.
6. HubSpot Service Hub: best for health organizations that already run marketing and sales on HubSpot
HubSpot's Sensitive Data feature lets Enterprise accounts, including Service Hub Enterprise, store health data in protected properties. Turning it on includes identifying as a HIPAA covered entity or business associate and accepting HubSpot's BAA. HubSpot's pricing page notes that its AI quality assurance and customer experience scores are unavailable for Sensitive Data accounts.
- Best for
- Health brands, clinics and digital health companies that want service tickets in the same CRM as patient marketing and sales
- Pricing
- Service Hub Enterprise is $150 per seat/month on monthly billing, annual pricing via sales, plus a $3,500 one-time onboarding fee. Customer Agent AI runs on HubSpot Credits.
- Strengths
- One CRM across marketing, sales and service, field-level encryption and access restrictions for sensitive properties
- Watch out for
- BAA requires Enterprise, and HubSpot's KB does not spell out which service tools sit inside it, so review the Sensitive Data terms. Some AI features are off for Sensitive Data accounts. See the HubSpot Service Hub alternative page.
7. Zoho Desk: best for teams already on Zoho apps
Zoho Desk publishes a HIPAA compliance guide. You can mark fields as ePHI, which encrypts them, restrict access with roles, profiles and field-level permissions, and export audit trails; Zoho stores audit logs for up to a year. Zoho provides its BAA template on request from its legal team. The guide does not say which editions support these features.
- Best for
- Small and mid-size clinics and health businesses that use other Zoho apps and want a low-cost helpdesk
- Pricing
- Free edition for 3 users, then Express, Standard, Professional and Enterprise, priced per agent/month (prices vary by region; check the pricing page in your currency). Zia AI from Professional.
- Strengths
- ePHI field marking with encryption, field-level permissions, audit trail export, broad Zoho suite
- Watch out for
- Ask Zoho which edition and which channels and AI features the BAA covers before you buy. See our Zoho Desk alternative page.
8. Help Scout: best for small email-first support teams
Help Scout lists HIPAA compliance as an add-on to its Pro plan, which lets teams store PHI in Help Scout. Every paid plan includes AI drafts, summaries and text editing, and AI Answers resolves questions on its own for $0.75 per resolution with a monthly spending cap.
- Best for
- Billing offices, digital health startups and small patient support teams that work mostly by email
- Pricing
- Standard about $21 and Plus about $38 per user/month billed annually ($25 and $45 monthly). Pro, where the HIPAA add-on sits, is sold through sales with a 10-user minimum. AI Answers $0.75 per resolution.
- Strengths
- Clean shared inbox, easy to learn, AI drafts on every paid plan, low per-resolution AI price
- Watch out for
- HIPAA only on Pro, which needs at least 10 users. No phone or SMS, so phones need another tool. Confirm that AI Answers is inside the BAA. See our Help Scout alternative page.
9. Spruce Health: best for small practices that need phones, texting and fax more than a helpdesk
Spruce is a communication app built for healthcare. Every trial and paid plan includes a BAA. It combines a medical phone system, SMS, secure app-based messaging, secure team chat, video and e-fax in one shared inbox, with AI transcription for voicemails and, on Communicator, call recordings.
For an independent practice, Spruce is often a better fit than any helpdesk on this list, including Aftersales, because the front desk lives on the phone and fax.
- Best for
- Independent and small group practices, therapists and direct primary care
- Pricing
- Basic $24 and Communicator $49 per user/month. A one-time $19.50 fee applies to enable outbound SMS.
- Strengths
- BAA on every plan and trial, phone, fax, video and secure messaging in one app, low cost per user
- Watch out for
- Not a ticketing helpdesk or contact center, with no AI agent that resolves conversations. Secure messaging requires patients to use the Spruce app.
See Agent on your own patient FAQs.Start a 14-day trial with test data, then talk to us about the Enterprise plan and a BAA before any PHI goes in.
How much does healthcare customer service software cost?
The price that matters is the plan that includes the BAA, plus AI usage. Here is an example with list prices.
Example assumptions: a patient services team of 10 agents handling 4,000 conversations a month, with AI resolving half (2,000 resolutions). Annual billing.
- Aftersales Enterprise: custom seat pricing from sales (the BAA is on Enterprise only), plus 2,000 Agent resolutions at $0.99 = $1,980. For reference, Scale seats list at $109 ($1,090 for 10) but Scale does not include a BAA.
- Zendesk Suite Professional: 10 agents at $115 = $1,150, plus the Advanced Compliance add-on (quoted) and AI agent resolutions beyond the included 5 per agent ($2.00 each pay-as-you-go, about $3,900 for 1,950 extra).
- Talkdesk Healthcare Experience Cloud: 10 users at $225 = $2,250 before AI agent usage, which Talkdesk quotes.
- Salesforce Health Cloud Sales & Service Core: 10 users at $350 = $3,500 before Agentforce and implementation.
- Spruce Communicator: 10 users at $49 = $490, with no AI resolution and no ticketing.
Check whether each vendor's AI is inside its BAA before you count its resolutions. If it is not, the realistic comparison is seats only, with staff handling every conversation. See Aftersales pricing for every plan.

How do you set up a helpdesk safely for patient data?
Whichever tool you pick, these steps keep patient data where it belongs.
- Sign the BAA before any PHI enters the tool, including during a trial. Use test data until then.
- Get the covered-services list in writing and switch off any channel, AI feature or marketplace app outside it.
- Turn on SSO, least-privilege roles and audit logs, and set a retention period.
- Verify identity before discussing an account, and share only what the conversation needs.
- Route clinical questions to licensed staff. AI and non-clinical agents should handle administrative topics only.
- Review conversations every week for anything that should have been escalated, and update procedures and macros.
