Why do generic helpdesks struggle with telehealth support?
Telehealth tickets are tied to visit start times, so a first-in, first-out queue puts them in the wrong order; clinical ones must also reach licensed staff.
- Visits are time-boxed. A connection problem is worth solving two minutes before a slot and close to worthless twenty minutes after. First-in, first-out queues bury it behind yesterday's billing questions.
- Clinical and admin messages arrive tangled. “My refill hasn't arrived and I feel dizzy” is a pharmacy question and a clinical one. Reps need routing that moves the clinical half to licensed staff immediately.
- Location decides who can treat. Telehealth is generally treated as delivered where the patient is, so clinicians usually need a license in that state, as HHS explains. A patient on a work trip becomes a scheduling problem.
- Coverage rules keep moving. Congress has extended Medicare's telehealth flexibilities several times; check CMS's Medicare telehealth page for the date that applies now. Each change brings a wave of “is my visit still covered?” questions.
- Protected health information is in nearly every ticket. Defaults that email full messages to staff or sync contacts to marketing tools must go.
- One problem, several parties. A missing prescription can involve patient, prescriber, pharmacy and payer, so reps need side conversations with each party inside the same ticket.
How should a telehealth support inbox be set up?
Build queues around what the patient needs and how soon, then route on intent, time to visit and the patient's state. Aftersales Inbox gives you views and SLAs, plus round-robin assignment on Growth and above, with custom fields such as visit type as filters and columns.
| Queue | What lands there | SLA target | Owner |
|---|---|---|---|
| Visit-day access | Connection problems and late clinicians, visits within 60 minutes | First reply within 2 minutes in visit hours | Virtual care coordinators |
| Scheduling and eligibility | Bookings, reschedules, out-of-state questions, employer or plan eligibility | 1 business hour | Patient support reps |
| Prescriptions and pharmacy | Not at the pharmacy, pharmacy changes, refill requests to route | 4 business hours | Pharmacy liaison |
| Clinical triage | Symptoms, side effects, results, crisis language | Set by your medical director | Licensed nurses or clinicians |
| Billing and insurance | Copays, denials, refunds, self-pay estimates | 1 business day | Billing specialists |
| Records and documents | Records requests, visit summaries, work notes, superbills | Acknowledge in 1 business day; finish well inside 30 days | Records lead |
| Privacy and complaints | Care complaints, privacy concerns, legal language | Same business day | Patient advocate, privacy officer |
Illustrative starting targets, not industry benchmarks; adjust them to your visit model and staffing.
Routing rules to add on day one
- Any patient with a visit in the next 60 minutes jumps to visit-day access, whatever the channel.
- Symptom and crisis keywords route to clinical triage and notify the on-call clinician.
- The patient's state at visit time decides which scheduling team owns the ticket.
- Records requests get a due date 30 days out, following HHS guidance on the right of access, with an alert on day 20.
- Uninsured and self-pay patients asking about cost get the good faith estimate path the No Surprises Act entitles them to.
Side conversations in Slack let a rep ask a prescriber or billing a question without leaving the ticket, and Workflows apply these tags, assignments and SLAs automatically.
Which reply templates does a telehealth support team need?
Write macros where one wrong word causes harm: clinical questions, crises, missing medication and money. Your medical director and privacy officer should sign them off.
Clinical question sent to support
Hi {first_name}, thank you for letting us know. Questions about symptoms and medication have to be answered by a clinician, so I've passed your message to our clinical team and they will reply here by {clinical_reply_time}. If you feel worse or think this is an emergency, call 911 now.
Crisis language
{first_name}, I'm glad you reached out. If you are in danger right now, please call 911. You can also call or text 988 at any hour to reach the Suicide and Crisis Lifeline. I'm bringing in a member of our clinical team now and I'll stay here until they join.
Prescription not at the pharmacy
Hi {first_name}, your prescription went to {pharmacy_name} on {sent_date} at {sent_time}, and new ones can take a few hours to process. I've asked our team to check with the pharmacy directly and I'll update you by {update_time}. To use a different pharmacy, reply with its name and address and a clinician will review the change.
Missed visit after a technical problem
Hi {first_name}, I'm sorry your visit on {visit_date} didn't connect. That's on us, so there's no missed-visit fee. I've held {slot_1} and {slot_2}; reply with whichever suits you. This two-minute device check catches camera and microphone problems before the visit: {device_check_link}.
Unexpected bill or denied claim
Hi {first_name}, thanks for sending this. The {amount} charge on {charge_date} is for your {visit_type} visit. {payer_name} processed the claim as {claim_status}, which is why the balance came to you. Our billing team is reviewing it and will reply by {review_date}.
Patient traveling to another state
Hi {first_name}, our clinicians must be licensed in the state you're in during the visit, and on {visit_date} you'll be in {travel_state}, where we {coverage_answer}. We can move you to {next_slot}, after you're home. If you need care sooner, {local_care_option}.
How should a telehealth support team be staffed?
Staff around visit times rather than office hours, and keep non-clinical support separate from clinical triage, each with a named owner.
- Patient support reps take scheduling, accounts and general questions.
- Virtual care coordinators own visit-day access, working off the clinicians' schedule.
- Billing specialists take insurance, refunds and estimates.
- Licensed clinical staff own the triage queue. Clinical leads can review and approve templates from free Lite seats: 20 on Growth, 50 on Scale.
Hours, peaks and seasons
- Cover every hour you offer visits, plus 30 minutes either side. On-demand urgent care needs support whenever its clinicians are working.
- Expect bursts just before each block of visits, and on Monday mornings as weekend questions arrive.
- Fall and winter respiratory illness season usually lifts urgent-care visits, and support volume follows.
- January brings new insurance cards, reset deductibles and eligibility mismatches, so add billing cover early in the year.
- Older patients are heavy users. In CDC survey data for 2021, 43.3% of adults aged 65 and over had used telemedicine in the previous 12 months, against 37.0% of all adults, so keep the phone line staffed through every visit block.
Where does AI help a telehealth support team?
AI takes drafting, summarizing and translating off your reps, so their time goes to conversations that need judgment.
- Copilot drafts cite their sources, so a rep answering “is my visit still covered?” can check the Medicare FAQ or plan policy behind the reply before sending. Teams using Copilot on every conversation see a 2.4× faster median reply (Aftersales' own figure).
- Summaries let a coordinator pass a ticket to a nurse without the nurse reading twenty messages.
- Translation in 31 languages, including right-to-left scripts, lets a rep answer a patient writing in a language the rep doesn't speak.
- Monitors check every conversation against rules you write, such as “no support reply contains dosing guidance”, instead of a weekly sample.
- Agent takes the routine share. The telehealth AI customer service guide has the conversation map and an example procedure.
Copilot is $29 per rep per month on annual billing, and every plan includes 10 Copilot and 10 translation conversations per rep each month.
Which metrics should a telehealth support team track?
Track whether patients got into their visits and whether clinical messages reached clinicians fast. Insights reports resolution rate, cost per conversation and CSAT; tags and custom fields cover the rest.
| Metric | Formula | Target guidance |
|---|---|---|
| Visit-day first reply time | Median minutes from a visit-day message to the first reply | Shorter than your late-join grace period |
| Tech-related missed visits | Missed visits with a logged technical issue ÷ scheduled video visits | Falling month on month; split by device and browser |
| Clinical handoff time | Minutes from a clinical message arriving to a clinician acknowledging it | Set by your medical director; review every breach |
| Records turnaround | Days from request to fulfillment | Every request inside 30 calendar days; report exceptions |
| Contacts per completed visit | Support conversations ÷ completed visits | Track by visit type; a rise points at a broken step |
| CSAT, AI and human | Satisfied responses ÷ all responses, split by who resolved | Compare both on the same conversation types |
These targets are deliberately relative; set absolute numbers from your own first 60 days of data.
How do you move a telehealth team to a new helpdesk?
Telehealth teams usually arrive from a shared Gmail or Outlook inbox, patient portal messaging, a separate phone system, or a general helpdesk such as Zendesk, Freshdesk or Salesforce Service Cloud. Move in this order so no patient data reaches the new tool before the BAA is signed:
- Sign the BAA first. No patient data moves before it exists; on Aftersales that means Scale.
- Decide what belongs in the medical record. Clinical messages should live in your EHR. The helpdesk routes them and shouldn't become a second chart.
- Map queues and SLAs from the table above, import your macros and rewrite any that drift toward advice.
- Move channels one at a time: email, then chat, SMS and phone numbers.
- Migrate history with help. Most teams finish in an afternoon, and Aftersales runs the migration with you on Growth and Scale.
- Keep the old tool read-only for a month for history lookups.
Not ready to move the whole team? Agent can run inside your current helpdesk first; the AI customer service for telehealth page explains how. Compare tools in the best customer service software for healthcare, or see other sectors in our helpdesk setup guides.
